• Department Circular No. 2022-0492 – Advisory on the Proof of Submission to the National Database of Human Resource for Health Information  System (NDHRHIS) by the DOH Regulated Health Facilities

• Effective June 17, 2022, the Health Facilities and Services Regulatory  (HFSRB) and Centers for Health Development – Regulation Licensing and Enforcement Division shall follow the revised Application Form 1, Form 2, both revision 02, and Application for PTC revision 07 to include the regulation of Cancer Treatment Facility based on AO. No. 2022-0012 dated May 19, 2022, entitled “Rules and Regulations Governing the Licensure of Cancer Treatment Facilities in the Philippines”. The said documents must be available at the point of use.

• Effective June 17, 2022, the Health Facilities and Services Regulatory  (HFSRB) and Centers for Health Development – Regulation Licensing and Enforcement Division shall utilize Order of Payment Form OP 01 (HB-PVT) revision 10, Order of Payment Form OP 02 (HB-GVT) revision 09, Order of Payment Form, OP 03 (OSS-NHB-ANCILLARY) revision 08, and Order of Payment Form OP 06 (CON_PTC) revision 08, consecutively to include payment for Cancer Treatment Facility based on AO No. 2022-0012 dated May 19, 2022.

• Effective February 14, 2022 the HFSRB and CHD RLEDs shall follow the Revised Notice of Violation Form rev 03, to indicate directly the type of health facility on the space provided during monitoring or surveillance visit. The said form must be available at the point of use.
