TO SUCCESSFUL PASSERS OF THE WRITTEN LICENSURE EXAMINATION FOR MASSAGE THERAPIST
DATE: April 19, 2022

 

Please be informed that a practical examination will be conducted at the following:

  • Metro Manila, Region 3, 4A and CAR – April 25-28, 2022, Venue to be announced Around Metro Manila
  • Davao Region – April 20-21, 2022, Venue Sunny Point Hotel – 818 Don Julian Rodriguez Sr. Ave., Ma-a Rd, Davao City, Davao Del Sur
  • Central Visayas Region – April 19-20, 2022, Venue at Sarrosa International Hotel & Residential Suites – Ayala access road (axis), Kasambagan, F. Cabahug St, Cebu City, Cebu
  • Western Visayas Region – April 19-20, 2022, Venue at Casa Pilar Beach Resort – Station 3, Boracay, Malay, Aklan (Kindly coordinate with CHD region 6 and training providers for the travel requirements)
  • Mimaropa Region (Palawan Examinees Only) – April 19, 2022, Venue at Dolce Vita Hotel – Villa Romasanta Street 4, Wescom Road San Pedro, Puerto Princesa

 

Time :

7:00AM – 8:00AM – Registration

8:00AM – Onwards – Oral/Practical Exam

 

Every examinee is advised to bring a subject.

Documents to be prepared/to bring:

  • Any government ID card as proof of Identity
  • Vaccination card or RT PCR test with a negative result (examinee and subject)
  • Also, please bring the following:
      1. Two copies of the passport-size picture with white background and Nametag
      2. Wear your uniform/scrub suit or white shirt
      3. Drape/s
      4. Disinfectants (i.e. alcohol, Lysol)
